Who Pays for this service?
Community Long Term Care (CLTC) under the community choices waiver and you will need to contact South Carolina Department of Health and Human Services.

The e-referral is the preferred method and most efficient however individuals may also make referrals by phone:

South Carolina Department of Disabilities and Special Needs (DDSN) under the IR/RD the Intellectual Disabilities/Related Disabilities waiver or you can also receive support through the Community Supports waiver.

How do I apply for Medicaid?
To apply for Healthy Connections, you will need to complete and submit an application. You may apply online at apply.scdhhs.gov. Please contact their help line at 1-800-726-8774 or visit them online at scthrive.org if you have questions or need assistance. You may also apply in person at your local office.
I already have Medicaid for my loved one, what do I do next?
You will need to contact either CLTC or DDSN to enroll in the waiver programs. You are given the option to contact either service to receive support.
What is the criteria to attend Club Horizons?
You must be at least 18 years old and meet the criteria for the waiver.
Will you provide Transportation and how much will that cost?
Yes! We will provide door to door transportation and transportation is covered under your support waiver.
What are your hours of operation?
We will be open year-round from 7:30AM to 5:30PM Monday through Friday. We will be closed for a few select holidays throughout the year.
What will be the staff to client ratio?
The state requires a minimum of 1:8 staff to client ratio; however, Club Horizons will have a 1:6 staff to client ratio.
My loved one requires medical supervision, how can Club Horizons accommodate our needs?
We will have a full time Registered Nurse onsite.
Can my loved one come to Club Horizons just a few times a week?
